Evidence collected. The auditor reviews the aspects register (last updated six months ago) and the waste transfer notes for a chemical waste stream that started two months ago, and interviews the process supervisor. Before raising a finding, the auditor checks whether the new waste stream was assessed in another controlled record, such as a change request, and confirms it was not.

Weak finding. "The aspects register seems outdated." This states an opinion without linking evidence to a requirement, so management cannot act on it.

Proper finding. "The aspects register (Rev 4) does not include the chemical waste stream introduced two months ago, although procedure EMS-P-03 requires a review when a new waste stream starts. Waste transfer notes WT-118 to WT-124 confirm the stream is active. The procedure's review requirement has not been met."

Follow-up. The auditee makes the correction by updating the register and evaluating whether the new aspect is significant. The auditee then identifies why the change did not trigger a review, for example a missing link between the change process and the aspects procedure, and takes corrective action. Within the agreed timeframe, the auditor verifies implementation and evaluates whether the corrective action addresses the identified cause. Where subsequent process changes have occurred, the auditor may sample those records to assess effectiveness.

ISO 14001:2026, the fourth edition of the standard, was published on 15 April 2026. It replaces ISO 14001:2015 and its 2024 climate change amendment, which are now withdrawn.
